            Health Insurance Certificate is required for your visa extension [ URL "LFHKEN-1.html#null

            arranged for in the Czech Republic ONLY!
            It has to cover at least the period of your visa extension. (Please note that the length o
            extension is dependent on the period you are insured for.)

            The conditions of the health insurance contract must cover treatment interventions in conn
            - injury
            - sudden illness

            - payment ability in context of repatriation to the country which issued your passport
            The health insurance must be of a value of at least 60.000,- EUR.

            The contract must contain information where and when it was signed.
