            The Certificate of Non-Membership is  provided by the Czech Medical Chamber (ČLK).

            You may provide the application including an officially verified copy of the Diploma this 
             


            • by e-mail – you have to send scanned copies of filled Application form [ URL "LFHKEN-370
              application_for_the_certificate_of_non_membership_in_the_czech_medical_chamber_1.pdf"] a

              officially verified copy of the Diploma) to the e-mail address specified below. You will
              information regarding payment of the fee (300,- CZK) – the bank account information, etc
              will be received by ČLK, the certificate will be prepared for you immediately and posted

              you provided on the application
              The certificate is provided in several languages (including English) – tick on the appli
              language you prefer.

            !!!Even if you apply personally you will not get the certificate straight away, it will be

            within a few days!!!
